Distribution Request Form
Through the Significance Foundation, donors are able to recommend a distribution be made to a particular charity or non-profit by completing a Distribution Request Form. The form will ask for basic information about the requested donation. Donors completing the form can print out the form, complete it, and fax it back to The Significance Foundation for processing.
